I can answer the second one : yes, magma builds up as more and more is forced to the surface. The pressure builds under the plug that formed in the last eruption and some magma usually finds its way out though little holes etc. In the end the pressure gets too great and the whole plug blows off releasing the pressure and all magma and stuff
